NOTICE OF PRIVACY PRACTICES

Orange County Social Services is currently in charge of administering the County Indigent Health Care Progam.  Individuals interested in applying for assistance may contact the office to request an application.  Upon request, an application with an appointment notice for an interview will be mailed.  The eligibility requirements are based on available income and resources.  If an individual qualifies for assistance, medical and prescription assistance will be offered. 

Below is a list of information required to complete an interview:

ORANGE COUNTY INDIGENT HEALTH CARE/MEDICAL ASSISTANCE PROGRAMS

Do not mail in application, bring it with you to your scheduled interview.  If you need assistance with your application please arrive 15 minutes earlier than your scheduled appointment.

In order to process your application you must bring the following information to your interview:

• Completed application

• Proof you are an Orange County resident such as:
  
  A. Drivers' license
  B. Mail addressed to you at your current address
  C. Voters' registration

• Social Security cards for all household members

• Proof of income for all household members such as:
  
  A. Check stubs for past 90 days or statement from the employer.  If self-employed, your records.
  B. Copy of SSI, Social Security, AFDC, Child Support, Workman's Comp
  C. If paid in cash, bring a statement from the employer

• A current bank statement -- checking and/or savings

• Savings account passbooks

• Most recent Income Tax Return (Copy of original)

• Copies of all insurance policies -- hospitalization, life, and burial

• If you are unemployed you must show proof of how you are living, proof of your last employment period (letter from the employer)

• Proof you applied for unemployment benefits and registered with "Job Workforce"

• If you have a lawsuit pending, we must have a statement from your attorney as to the status of the case and record of any money advantage

• If you have applied for SSI/SSD submit proof

• Proof of food stamps

• If you are divorced or legally separated, submit proof
